PURPOSE OF THIS DOCUMENT<\/strong><\/h2>\n\n\n\n<p>This document ensures that clients of Ayur Health Ayurveda Centre understand:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>the nature and scope of Ayurvedic treatments<\/li>\n\n\n\n<li>the potential risks and limitations<\/li>\n\n\n\n<li>the responsibilities of both practitioner and client<\/li>\n\n\n\n<li>the requirement for accurate disclosure of medical information<\/li>\n\n\n\n<li>the boundaries of Ayurvedic care as a complementary therapy<\/li>\n\n\n\n<li>circumstances requiring referral or medical attention<\/li>\n<\/ul>\n\n\n\n<p>By signing this form, the client confirms that they have read, understood, and voluntarily consent to treatment.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>2. NATURE OF AYURVEDIC TREATMENT (Complementary Therapy Disclosure)<\/strong><\/h1>\n\n\n\n<p>I understand and acknowledge that:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Ayurveda is a <strong>traditional and complementary health system<\/strong>, not a substitute for medical diagnosis, treatment, or emergency care.<\/li>\n\n\n\n<li>Ayurvedic practitioners at Ayur Health are <strong>not medical doctors<\/strong>, and Ayurvedic treatments are <strong>not intended to diagnose, cure, or treat illnesses<\/strong> as defined under Australian medical legislation.<\/li>\n\n\n\n<li>Ayurvedic recommendations, including Panchakarma, lifestyle practices, herbal formulations, and dietary guidance, are based on traditional knowledge and may not be validated by conventional biomedical research.<\/li>\n<\/ol>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>3. SCOPE OF AYURVEDIC SERVICES PROVIDED<\/strong><\/h1>\n\n\n\n<p>The treatments offered may include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ayurvedic consultation and assessment<\/li>\n\n\n\n<li>Panchakarma therapies (e.g., Snehana, Swedana, Virechana, Basti, Shirodhara)<\/li>\n\n\n\n<li>Herbal preparations<\/li>\n\n\n\n<li>Lifestyle, dietary, and wellness recommendations<\/li>\n<\/ul>\n\n\n\n<p>Each therapy will be discussed prior to commencement.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>4. CLIENT RESPONSIBILITY TO DISCLOSE HEALTH INFORMATION<\/strong><\/h1>\n\n\n\n<p>I understand that Ayurvedic therapy requires accurate disclosure of:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Full medical history<\/li>\n\n\n\n<li>Current diagnoses, symptoms, or conditions<\/li>\n\n\n\n<li>History of liver, gallbladder, hormonal, metabolic, or neurological conditions<\/li>\n\n\n\n<li>Mental health concerns, including anxiety, depression, trauma, ADHD, difficulty concentrating, or dissociation<\/li>\n\n\n\n<li>Current medications, supplements, or herbs<\/li>\n\n\n\n<li>Pregnancy status, menstrual irregularities, or fertility treatments<\/li>\n\n\n\n<li>Recent injuries, hospitalisations, or surgeries<\/li>\n\n\n\n<li>History of adverse reactions to herbs or cleansing protocols<\/li>\n<\/ul>\n\n\n\n<p>I understand that <strong>failure to disclose accurate information may put my health at risk<\/strong>, and the clinic is not liable for outcomes resulting from incomplete or misleading information.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>5. INDIVIDUAL VARIATION &amp; LIMITATIONS<\/strong><\/h1>\n\n\n\n<p>I acknowledge that:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Outcomes vary depending on age, constitution (Prakruti\/Vikruti), lifestyle, past medical history, stress levels, and compliance with recommendations.<\/li>\n\n\n\n<li>No specific results are guaranteed.<\/li>\n\n\n\n<li>Panchakarma and cleansing therapies may temporarily aggravate symptoms as part of the detoxification process.<\/li>\n\n\n\n<li>Emotional sensitivity, fatigue, digestive changes, or heightened stress responses may occur during treatments.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>6. RISKS AND POSSIBLE ADVERSE EFFECTS<\/strong><\/h1>\n\n\n\n<p>While generally safe when delivered appropriately, Ayurvedic treatments may involve risks, including but not limited to:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>General Risks<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>fatigue, dizziness, headaches<\/li>\n\n\n\n<li>abdominal discomfort or digestive changes<\/li>\n\n\n\n<li>emotional release or heightened sensitivity<\/li>\n\n\n\n<li>temporary aggravation of symptoms<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Ghee (Snehapana) and Cleansing Protocols<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>nausea or digestive heaviness<\/li>\n\n\n\n<li>intolerance to fats<\/li>\n\n\n\n<li>abdominal discomfort in clients with liver\/gallbladder issues<\/li>\n\n\n\n<li>risk of aggravation if taken in high doses or without proper compliance<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Swedana (Steam Therapy)<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>overheating<\/li>\n\n\n\n<li>dehydration<\/li>\n\n\n\n<li>lightheadedness<\/li>\n\n\n\n<li>skin irritation or discomfort<br>Clients must inform staff immediately if they feel too hot, faint, or unwell.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Shirodhara<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>overstimulation or emotional sensitivity<\/li>\n\n\n\n<li>temporary headaches or dizziness<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Herbal Medicines<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>allergic reactions<\/li>\n\n\n\n<li>interactions with medications<\/li>\n\n\n\n<li>digestive upset<br>I must inform my practitioner of all medications I am taking.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Mental &amp; Emotional Health<\/strong><\/h3>\n\n\n\n<p>Clients with anxiety, PTSD, dissociation, burnout, or mood disorders must be aware that intensive therapies may feel overwhelming or activating.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>7. CONTRAINDICATIONS AND SAFETY PRECAUTIONS<\/strong><\/h1>\n\n\n\n<p>I understand that Panchakarma and cleansing therapies may <strong>NOT<\/strong> be suitable if I have:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>liver disease (including non-alcoholic fatty liver)<\/li>\n\n\n\n<li>gallbladder disorders<\/li>\n\n\n\n<li>severe digestive weakness (low Agni)<\/li>\n\n\n\n<li>uncontrolled hormonal conditions<\/li>\n\n\n\n<li>acute infections<\/li>\n\n\n\n<li>unmanaged mental health conditions<\/li>\n\n\n\n<li>significant fatigue or systemic depletion<\/li>\n\n\n\n<li>recent trauma or nervous-system instability<\/li>\n<\/ul>\n\n\n\n<p>The practitioner may postpone or modify treatment for safety reasons.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>8. CLIENT AGREEMENT DURING TREATMENT<\/strong><\/h1>\n\n\n\n<p>I agree to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>follow instructions provided for preparation, treatment, and aftercare<\/li>\n\n\n\n<li>communicate any discomfort or adverse reactions immediately<\/li>\n\n\n\n<li>avoid self-adjusting doses of herbs, ghee, or treatments<\/li>\n\n\n\n<li>attend recommended follow-up consultations<\/li>\n\n\n\n<li>follow safety protocols during steam therapy and detox processes<\/li>\n\n\n\n<li>arrange medical care if advised to do so<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>9. MEDICAL REFERRAL POLICY<\/strong><\/h1>\n\n\n\n<p>I understand that:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If concerning symptoms arise (breathlessness, chest pain, severe abdominal pain, neurological symptoms, fainting, panic attacks, dissociation, or injury), I may be referred to a GP, emergency services, or another health professional.<\/li>\n\n\n\n<li>The clinic may pause or discontinue treatment in the interest of safety.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>10. LIMITATION OF LIABILITY<\/strong><\/h1>\n\n\n\n<p>I acknowledge that:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ayurvedic services are provided under Complementary Medicine Australia (CMA) guidelines and Australian consumer law.<\/li>\n\n\n\n<li>Ayur Health Ayurveda Centre and its practitioners are <strong>not liable<\/strong> for adverse outcomes arising from:\n<ul class=\"wp-block-list\">\n<li>non-disclosure of health information<\/li>\n\n\n\n<li>failure to follow instructions<\/li>\n\n\n\n<li>aggravation due to pre-existing conditions<\/li>\n\n\n\n<li>reactions beyond reasonable clinical foreseeability<\/li>\n\n\n\n<li>self-administered herbs or treatments not provided by the clinic<\/li>\n\n\n\n<li>external treatments undertaken concurrently without disclosure<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>11.
